Insurance contracts vary between these entities, and some insurance plans may be accepted under one practice but not the other. Completion of this form allows us to review your information and determine scheduling options, provider availability, and potential insurance coverage prior to an appointment.

Sonder Psychiatric Services, LLC and Holly Neuman, LLC participate with select insurance networks. However, participation with an insurance company does not guarantee participation with every insurance plan offered by that company. Network participation may vary based on factors including, but not limited to, employer-sponsored plans, marketplace plans, Medicaid products, Medicare products, third-party administrators, delegated networks, and specific benefit structures.
